The doctors and staff at Brook Medical Centre in Stoke-On-Trent are proud to offer the highest standard of patient-centred healthcare.
We run many clinics for the management of chronic diseases such as asthma and diabetes and offer a wide variety of other medical services including antenatal and postnatal care, minor surgery, childhood vaccinations and well-person check-ups.

Spring Bank Holiday
The surgery will be closed on Monday 27th May. We re-open as usual on Tuesday 28th May.
In the case of an Emergency please ring 838355. In the event of chest pain, etc, please ring 999.
From Tuesday March 12th all out-of-hours calls will be taken by the new NHS 111 service. This is for people who need medical help or advice urgently, but it's not a life-threatening situation / 999 emergency.
Instead of out of hours calls being diverted to North Staffs Urgent Care at Basford, patients who require out of hours advice or consultations will be asked to phone 111.
We are told that it should be a fast and easy way to get the right help.
Calls are free from landlines and mobiles.
Text Message Communication
We are now able to send automated text messages giving confirmation of appointments made and a text reminder a day before. This will be for a limited type of clinics initially, e.g. some annual review clinics.
We will be pro-actively requesting consent for patients to receive this service on an opportunistic basis. As we already have many mobile phone numbers in our records, it would be impractical to try to obtain consent in any other way.
The practice transmits only the time, date and place of an appointment made, and not any information that would enable an individual patient to be identified, or clinical information such as test results.
Please fill in a consent form that is available at reception, or available below for download. If more than one person shares the use of the mobile phone, we will need a consent form from each of those people.

The Practice ran a Patient Survey earlier this year. The Report, with input from our Patient Reference Group (PRG), has been uploaded to this site. From the results of the survey we have put together an action plan. Click for more details
