New Patient Registration

The doctors welcome new patients who live within our practice area.

On requesting to join the Panel, people will be asked to complete a New Patient Questionnaire and make an appointment for a simple health check with our HCA who will ask about medical history, etc.

You must attend for the heath check to be officially registered with the practice.

Please note that incomplete information may delay your registration.

Please ensure that you attend your appointment as failure to attend will mean that you will not be registered with the practice and an important appointment will have been wasted.

Registration Application forms

The process to register is simple. Please follow the list below in order to make registering as simple as possible

There are two forms that need to be completed to register with the Practice.

• You can pick up the forms from the surgery and fill them in, or
• You can print them off from this page and fill them in (see links below)
• Book your appointment with the surgery.
• When you come for your New Patient Health Check appointment, please ensure you
  • Bring the completed forms with you
  • Bring the required items listed on the New Patient Health Check form

Please remember to make your appointment with the surgery. A member of staff will help you to book the appointment to your and/or your family's needs.

Please be aware that there may be occasions that you will be unable to register, such as living outside the practice boundary. In these cases the reason will be explained to you.

Non-English Speakers
